The rope itself is a clothes hanging line and it's pretty dirty. Even after putting soap on it it won't constrict enough to cut off circulation....should I wash it? Then try again? Perhaps I should secure it into place as well

Personally I would not trust a ligature unless it is a 10-12.5 mm strong synthetic rope like polyester or nylon.

It should be thicker IMO, like the thickness of a rock climbers rope minimum. Also, the knot looks wrong. I see a lot of Indian suicide hangings use long scarves and it would seem like they are very effective as the diameter serves to evenly spread out the constrictive force making for a far more comfortable passing.

Does thickness have an effect on success? Or is it a confort thing
