Depends on the context. It's a greeting. No more.
It's a question denoting concern. Answer accordingly.

Pretty much this, it definitely depends on the context and who's asking it. It doesn't really bother me if anyone asks me here. They have atleast some inkling of how I am.

its been a year now that i haven't answered that question irl.
every time i read or hear it i freeze and choke up.

Hate does not begin for me. Most of the time it is a plastic, could care less, question. A better question might be something like " how do you feel today?" Even that can be plastic. It takes alot of time beening around people to figure out the ones the are caring and the ones that go throught "the motions" I NEVER answer anyone who asks me that question. I just walk away.
