I know overdosing is considered a non method, but realistically a lot of people are still going to attempt it anyway, myself included. (If it doesn't work I will try ligature strangulation but could just as easily fail with that).

I am planning on trying 1500mg codeine with alcohol soon. Most of what I've read says this should be enough to take me out.

Is it always a good idea to get an antiemetic for any OD? If so what are the most available ones?
Is vomiting one of the main factors that causes OD attempts to fail?

If you drink it or consume it via your mouth then yes. If injected then no.
Most ODs fail because the drug is either not strong to cause damage, they get rescued, or just vomit it out.
Idk what the other reasons might be but these ones happen often especially the rescuing part.

Look up the SN method they talk about antiemetics and which one to use. Using the wrong dosage or wrong drug could make it fail.
Also double check everything especially when ODing is your main method.
I have no clue whats the lethal dose for codeine. You should do more research.
Idk it seems a bit low? but with alcohol maybe it's more lethal.

After reading some sections of the PPH, I'm gonna shelve this plan for the time being until I get meto.

According to the PPH, if I take 3 10mg meto pills 40 minutes before and smoke weed 20 mins before, that should effectively help deal with the vomit risk.

I might also get some diazepam and other opiates to throw into the mix.

Yeah, 1500mg codeine is easily supposed to be in the lethal range, and alcohol will make it much more dangerous.

If I attempt this, I am thinking of duct taping my mouth so I can't vomit lmao.

You can still vomit, it just won't leave your mouth. Unless you want to choke to death from your own vomit, then taping your mouth won't make a difference.
