Makes you wonder what situation would be so desperate that it overcomes SI entirely. Nothing really comes to mind. In the example of stage 4 cancer, my state at least would allow assisted euthanasia though it would take awhile to implement, so having expert assistance trumps doing something alone that would likely fail or be intolerably violent.
