asking for a crisis screening

i think i need a crisis screening but i dont wanna go alone. im debating asking my mom to come with me. (im 22) how should i word it to her?

my idea is "i think i need to get a crisis screening. like tomorrow or something. would you go with me please?"

I think being honest could be best, tell her about situation, but this is only my thought, maybe someone could came with better idea
