Is it really? I assume you're from Germany. You just have to write it yourself and sign it and it's valid.

I'm planning to just write: do not give me artificial breathing, feeding tubes, resuscitate me under any circumstances.

It's better than nothing.
For the binding agreement you have to talk to the doctor and need it signed by a lawyer. It's also only valid for 8 years and also becomes irrelevant should medical treatment improve after you signed it.

Is this part of your last will ?

If you're in Germany, then the doctors have to follow your Patientenverfügung. It's valid forever as soon as you sign it. (Unless you revoke it or you write valid until date xx.xx.xxxx in it)
