What's the best position to place the rope around the neck to close off the arteries with minimal discomfort? I have quite a large adam's apple and if I wrap anything around it, I push it backwards, making me become very nauseous.

Same problem here. I've found that placing the rope higher on the neck under your chin solves this issue, assuming you are talking about full suspension. Try gripping your throat (above the adams apple) and squeeze as hard as you can. Your vision should start to go dark and you will feel a little unsteady. Maybe don't do this while standing, just in case.
