If it all worked out, you'd ctb instantaneously on impact. If you panic and move or a piece of the locomotive or a cowcatcher cause a glancing impact then the damage could be almost anything. Losing a limb or limbs, brain injuries if the head is impacted etc. Depending on the ground clearance of the engine you may be caught beneath it and dragged.
As far as others are concerned, shock and trauma could be expected to the loco crew, accident and emergency workers who attend and railway employees who have to clear your pieces from the track and the locomotive.

Yeah, if your neck was on the track and a wheel hit it/ran it over, you'd be no more.

How do people get decapitated from a train? I feel like the cowcatcher would hit me off and I'd have to have someone wipe my ass for me for the rest of my life.

The train is going so fast and it's so heavy that it's not going to *bump* over your neck, the wheels will just go right through.

It's not so difficult to find successful suicide by train videos on gore sites.

India seems to be number one on this method.

You would probably panic and try to move at the last second and just end up with life-altering injuries. I think that's the main reason it's not talked about much, in addition to it being terribly traumatic for the driver. I think it's just not as good a method as it sounds.

I often watch gore videos of this scenario. Most of the time it's someone in India getting hit by a train.
This is actually not a bad way to go out. You'd just have to overcome SI and be still until the final moment. It would be painless and quick.
