I have bad reflux (GERD) for which I take PPI daily (esomeprazole 40 mg). I sleep in an inclined position with head about 30 cm higher than feet.

The shit medication I have been prescribed causes most of the reflux and when I was slimmer (BMI = 22) I didn't have any reflux.

Yesterday I didn't take PPI and threw up during the night even though I had fasted all day. It's common for me to get heartburn and throw up 1-3 times a week (during night at around 3 a.m) just eating normally but not very healthily.

So, given that SN is pretty bad gastric irritant, I am concerned I will not be able to take it at all, meaning instant heartburn and vomit. Could it help to use larger dose of PPI before SN?

I think it's worse for me because I pretty much lie in bed all day doomscrolling, don't exercise anymore and eat low quality carbs. When I did keto the gerd issues were much better.
