Don't fall in love with every person you meet. Don't be so stupid and naive and get out of your home town. Nothing good will come of staying around people you know. Don't have kids ever!
Never forget just when you start enjoying life and thinking you might actually matter, life will trample those stupid ideas and remind you not to get your hopes up because your just a pathetic annoyance that others only put up with when they think they have to.
And make sure when you get your chance at 19 to end it do it properly and don't fuck it up.
I'd give her such a big hug and tell her that none of what happened is her fault, that she's not some monster that deserves all the horrific shit that people did to her. I'd also tell her she's trans and to transition asap because that'll help the dissociation and will help things down the line

I'd say:
"Don't be afraid of talking about your problems with other people, you ain't a rock and if you will supress all your pain right now, you will hide it much deeper than you would like it to be and it will transform into a lifetime trauma you will be dealing with your entire childhood (or life). After moving to another city try to accommodate with new people. Don't pretend to be someone you are not.
Take care of your hygiene and start working from the very young age so you won't feel like you're completly dependent from your narcissistic mother.
DO NOT LET YOURSELF BE SOCIALY ANXIOUS. It will be an instant game over.
Do not escape from reality by playing some random mobile games. Start learning english earlier instead.
And always have your dignity, don't let anyone take it away from you. I would also hug him, tell him that I love him and that it has never been his fault.
