This is something I have vast experience with. I don't know if it is a contributing factor to CTB but I do know that 90% of the time it is very unsatisfactory. Do you guys also have experience hiring professionals?

But I speculate it is possible for them to be prone to mental health issues or substance abuse etc. as a result of working in their profession.

Via online classifieds websites, dating websites etc. In the EU it's pretty common, there are many free websites with an adult dating section and a separate one for sex workers. If you know how to read between the lines you should find someone corresponding to what you're looking for.
