If you put a bunch of weights on top of yourself in a bathtub and try to drown, would it possibly work? Its less of a sting compared to salt water in an ocean. But I suppose one would need to really stay down

The most painful CTB methods could be electrocution, self-immolation, or drinking highly corrosive liquids. In order to make drowning comparably painful, you'd have to drown yourself in something like a strong acid, lye, or very hot water. As long as drowning is done in normal water and under proper sedation, there should be no pain at all.

I can't think of anything better than gas asphyxiation, which produces fainting via hypoxemia like drowning, but much faster and without discomfort associated with aspiration of liquid. Finding at least one available gas that could be used for asphyxiation shouldn't be too hard, considering how many options are present in general (food-grade nitrous for whipped cream, inert gases for welding or other purposes (e.g., making nitro coffee), refrigerants, propellants in air dusters, fuel gases). As the last resort, it's possible to produce hydrogen in a reaction like
2 Al + 2 NaOH + 6 H₂O → 2 NaAl(OH)₄ + 3 H₂↑
(it may produce some aerosol, which should be filtered out before inhaling the gas).
As for OTC medication, I don't know anything that could be comparably good.
