My plan is to go camping for a few days, leave some frozen burgers in a cooler, set up the tent with the sleeping bag, and then have the grill running in the back seat of the car. Since the lid of the grill would be off and because the grill would be in the back seat instead of the trunk, I'm wondering if it would set off some red flags.

It depends on the weather. If it's crummy weather --you wanted to go camping but then got rained on-- it might pass. But if it's good weather... As @Severen says, who grills burgers from the back seat?

You're right, but I was hoping that people would just assume that I'm not the sharpest tool in the shed. I would leave the grill in the trunk, but I'm worried that I'll be more likely to survive with a killer headache and/or brain damage.

From doing some reading this is looking like an unreliable way that has a lot of suffering like suffocation. Thoughts?

In my opinion, this method is probably one of the most reliable and painless way to ctb. The execution of this method is where a lot of people (myself included) fuck up, but the same can be said about every other methods to ctb.
