Definitely psychological. Maybe I am lucky enough to not have had any more than a few broken bones, tooth infection, and body mods as my greatest pain experiences, but I have never had a physical pain experience that was something that made me want to CTB.
I'm actually not sure. I have had some pretty intense (for me) physical pain and some pretty intense recurring pain in my life although I've never wanted to ctb from it. But I also don't consider my psychological problems "pain". Even psychologically, to me "pain" has to do with some sort of injury or damage whereas, for me, I think my depression is just more of a profound sadness. It's not like something is "broken" or even not working correctly. It's more like this is a state that I am in. That doesn't really make sense but honestly this question has me a bit baffled.

I had both at the same time and all of it combined was completely agonizing. I had chronic pain every day for about 6 months and then my father reacted to it by screaming at me and basically telling me to kms. Which is only like the 2nd or 3rd time he's done that but oh well.
I can tolerate physical pain. I'd rather be in blinding agony then the mental torture I've been forced into. Give me the physical over mental pain all day. Frankly the world is far more understanding of physical pain anyways.
