I'm in the USA if that affects anything.

It had been a while since I've cut myself and my old scars are quite faint, but this weekend I've struggled more than usual. I really went ham on my left leg, totally forgetting I have a physical tomorrow. I can lie about the scars being old, but I have a LOT of cuts on my leg now, very obviously fresh. Will my doctor make me take my pants off? Can I decline? It's been years since I've seen a doctor due to lack of health insurance so I have no idea.

It depends on what kind of physical you're getting. The most I've had to do as far as dropping my pants, is when they want to grab my testicles and tell me to cough.

I guess that's a relief, as I don't have any. First time I've been grateful for that fact.

I just went for a physical for the first time as an adult, USA.

They do have a basic mental healthcare screening (depression) and do ask to feel various parts of the body for abnormalities, including exposing wrists and ankles by pulling off my socks. But no, I was not asked to remove my pants, I was wearing shorts and there was no examination of my thighs or calves.

However, as a child, I was told to undress and get into robes and everything was inspected, but at the same time I also had various skin conditions so it was kinda normal.

I'm not sure if I'm the norm though for an examination.

Even if doc asks you to, you can just say no. They might be huffy like "Why bother coming if you won't participate," sort of thing but yeah, no one is going to forcibly drop your drawers, you're good.
