Why did some SN cases die even after reaching the hospital?

I've read some cases of people who took SN and were found alive and transported to the hospital. And they died at the hospital. Why does that happen? Is methylene blue unavailable at some hospitals?
The methylene blue can't restore their blood fast enough and they have a heart attack. Out of oxygen and so out of time. There's only so much the doctors can do.

Most EDs have it, or can get it quickly. When that happens, it's usually timing. They were found alive sure, but the damage was already too far along by the time treatment started. Delays, high dose, complications, sometimes they don't know it's SN that's been taken. By the time they're in hospital, even with treatment and ICU care, it can be past the point where it's reversible.
