Questions about night-night

Does the belt or whatever you wrap around your neck have to be fairly large? I've got a small cloth band thing that can fit all the way around my waist, and I thought I'd tie an adjustable tension knot in it, and put the padding under it. For padding, can I just ball up some socks and use that, or does it have to be thicker? I spent two hours trying it last night to no avail. Also, is there any guide to finding the 'sweet spot?'
