if someone took fentanyl/heroin and placed their head in a good noose, it would be 99% guaranteed death,
if the noose and the anchor point is set up well enough.
i think in the case of using hard drugs, even with a partial hanging it would be 99%.

if it's a person who never took the drug before, then it wouldn't take much to make yourself pass out on it.
so hypothetically, you would just put your head in the noose, and then intentionally OD.

if the anchor point is strong enough and the rope set up right, then how would that NOT work?

Something that could go wrong is that one ruins the hanging setup while intoxicated but not yet unconscious. But that risk could be mitigated by using an ROA with a rapid onset, such as smoking or IV.

Full suspension is very likely to work, but I'm less certain that partial with OD would work too. Purportedly not much pressure is needed to compress the carotid artery and the jugular vein, so it might work if one is in a position to apply enough pressure and the rope is properly tied around the neck. But that will probably still be riskier than full suspension.

I don't know how important a consideration this is for you, but a big part of the appeal of combining hanging with OD is that the drugs might make the hanging painless by rendering one unconscious or otherwise incapable of feeling pain. It seems to me that with the right drugs and ROA hanging would be (almost) painless. But then it might also not. Hanging triggers instincts deeply rooted in our biology which might not be overridden by taking drugs.

If you got nauseous, which often happens with opioid ODs, and then had to vomit with your head in a noose and/or something squeezing your neck. Quite unpleasant.

Not doubting that, but, did this happen to you? Sounds terrifying.
