I need advise about how to end my relationship before ctb

Two days ago I finally had my diagnose. I have cyclothymia. Personally I don't want to be a burden. I want to ctb since I'm a teenager but now the desire is even stronger. I want to end my relationship before this happens, months before, so please, do you have any advise?
