Has anyone ever been able to get to the roof of a tall building? I'm thinking at least 10-stories high.

Honestly, I don't know where the stairs/doors to the roof of any buildings I do have access to even are.
Any ideas of where those are usually located in commercial buildings?

I'm assuming they have security measures to prevent people from accessing it, even if it's just a door with regular locks.
Does anyone know what those measures are and how to potentially bypass them?
