Method What position and ammo type has highest lethality (12 gauge shotgun)?

I'm going to do this, but need to be absolutely sure there is no chance or as little chance of survival, hence why I am going shotgun to head. I do not want to fuck this up. I have failed to kill myself over a dozen times in my life, and this is the only method I have not tried.
