I mean in any death there's risk of injury and handicap. Shit, you'll die and that's the point! If you are worrying about being injured if you survive then maybe you shouldn't attempt to ctb unless you are certain you won't.
That said, I don't even know how to describe "most effective to CTB" because it can either killer you or it can't.
Unless one can get their hands on some fentanyl (not all that hard to do these days), and supposing one doesn't have a tolerance… short of that, yeah, no, it's likely going to fail.

Look into how successful overdosing is- it is ridiculously low. It is far more likely to end up in hospitalization or whatnot. Trust me here, I've experienced it myself- but you can look up the stats. It is so unlikely to kill you.

You can still go for OD but you have to find the right drugs.
Rofl yes. If someone has absolutely no tolerance, perhaps me(t)h(a)done will do the trick. Still, an OD is not a good method. So very much can go wrong. Perhaps combined with another method as a failsafe- but as the primary means of ctb? It fails so far more often than it is successful, so I would personally not recommend it.

You can combine drugs that potentiate each other like opioid + benzodiazepine. The PPeH has a section on drug "cocktails". Another issue is vomiting, so you'll likely need a strong antiemetic as well.
