Is it whether there has been any attempt, or just when death is a distinct possibility? Should there be at least some preparation required before you consider it a genuine suicide attempt?

For example, I've "practiced" killing myself (choking myself with a cord, mixing alcohol with my medications), but never to the point of being rushed to the emergency room. These, at least in my opinion, were not genuine attempts.

But what do you all think?

Whenever the intention is to die by performing a method, this qualifies as an attempt. If not then it is most likely a gesture or some fetish

Yeah I guess anything you do intending and expecting to die by doing qualifies as an attempt

What does one gain from classing them as "Real" or not?

If the persons intent was to die, it's an attempt. If it wasn't, its not an attempt. The thing is though, if you died hanging from a noose, nobody knows the intentions you had anymore. Just because you didn't mean to die that time, doesn't make it a lesser action.

I suppose I'd have to agree with you. I was mostly thinking about it as suicidal ideation or spontaneous suicide. More along the lines of 'Oh, how I wish I could do it...but I can't.' 'I'm too scared.'
