I'm unsure what if the "blank" was meant intentionally, I am under the impression and blank wouldn't be fatal ? (I don't really know though).

However, assuming a loaded shotgun, according to Lethality-Time-Agony Method Calculator, shotgun is the highest lethality of the common methods, rated even higher than explosives.
