As per title, have uncertainty health and mind rapidly declining, to ctb or not?

Only you can answer that question or not. If you have certainty that's your choice, I'd say just be sure you have that first because when it's done it's done. If you are having any doubts, I'd say show yourself the respect you deserve and take a moment to think on it. Find things you like to do to really focus on and clear your head out when you can, and try to organize your thoughts between doing those things. Music you like, any hobbies, whatever. Sometimes you need that break for things for a bit. Sorry you're struggling in any case. You're in my thoughts. For as long as you decide to stay and talk, I'm around so don't be a stranger if you feel up to it.

Of course I would wish to if possible, I would wish to cease existing under any circumstances personally as life is just an unnecessary harm but after all as it's your existence so only you can decide what to do.

I think one important question is to ask yourself, if your deteriorating health would prevent you from killing yourself in the future. I have known several persons who had thoughts about ctb when diagnosed with cancer, but put it off until they were no longer physically capable to ctb and then regretted that they hadn't done it while they were still able to. So if that might be how you will end up you should consider, if being left with unbearable pain with no way out would be preferable to killing yourself too soon.
