I was wondering how can full suspension hanging fail ? What can go wrong and why people fail ?

Rope/rope support breaks, someone comes along to rescue you, the only 2 things I can think of…

1. Rope not secured tightly enough

2. Person is found before they fully die (around 30-40 minute mark)
