If you are heavily injured and in a lot of pain while having something that can stop your suffering immediately, then self-preservation may be the last thing you'd be concerned about. Your brain wants to stop the immense pain in the first place, and if you realize that death stops it, then suicide may be automatically deemed a good remedy. Discomfort avoidance doesn't care about survival, it motivates you to resolve discomfort by any possible means. It's only a matter of circumstances whether prevention of discomfort also prevents potentially lethal self-harm or does the exactly opposite thing.
It's more of an exception, most people continue to live no matter what.

I don't think I've ever experienced SI taking over during any life experience. My first and only real attempt, no SI were present. But I was trying to overdose so maybe that's why. Usually I feel very in control of my brain and my body whenever there's adrenaline involved. I don't shy away from high cliffs or dangerous climbs on hikes because I like the way my body screams at me to stop. It's a very addicting feeling, and it momentarily satiates my desire for suicide.
