Any one here know of any failed N attempts due to vomit despite taking anti-emetics correctly? I am considering Domperidone + Ondansetron and the ahead regimen.

Below is from some version of PPH

"If vomiting does occur, the individual should bring up (vomit up) as much of the drug from their stomach as they can and the attempt to end their life should be abandoned. Ipecac Syrup can be used to encourage vomiting. It may be advisable to have some on hand. Ipecac can be readily obtained from the local pharmacy."

but the 03/19 version of PPeH does not mention Ipecac and suggests using fingers down the throat instead. Does anyone know why this change?

How best to ensure most of the N is removed from stomach assuming one is still awake?

Is it possible to vomit after passing out assuming one passes out?

I can't answer all your questions, but can help with some.

I'm not familiar enough with this method to answer. Hopefully someone else can advise.

Not sure. Cost maybe? However, fingers down the throat is not reliable for those individuals that have, for whatever reason, overcome their gag reflex.

Vomit as much and as quickly as possible. Both Metoclopramide and Domperidone are gastro-prokinetic, so increase the rate of emptying of the stomach contents into the intestinal tract, which is where absorption occurs.

Is it possible to vomit after passing out assuming one passes out?
Absolutely. Many (most?) deaths from drug OD result from choking on vomit whilst unconscious.

Wait, why would you want to vomit out the N after vomiting? Is it because, if you don't then the N will just make you really sick and not end your life? So then you gain nothing except becoming sick?

@Severen pretty much, yeah. If you vomit after introducing it to your system it's likely that you've lost some in the process of vomiting, which might put you under the required lethal dosage (or at least dosage for desired effects). At that point it's better to get as much out of your system as quickly as possible instead of letting your body absorb all of whatever remains if you're looking for a peaceful death. I'd have to do more research to be able say much more on the subject, though (like what your chances are of dying after vomiting and all that).

My understanding is that vomiting after N is unusual. Is that correct?

According to wikipedia, Ipecac syrup use is no longer recommended for inducing vomiting and purging poisons from the body because it can often be a poison by itself and cause side effects that may complicate treatment of poisoning by doctors. There are also studies that show that ipecac syrup is terribly innefective in removing the poison from the stomach compared to activated charcoal.

If this happens, is it best to just phone 911 and tell them what you took? Don't want to risk brain damage.

Also: just to have all bases covered, let's say this happens. I'm assuming that the doctors might ask you about acquiring barbiturates. Could you just refuse to answer their questions? Are you obligated to answer?

Entirely your call, but that's going to guarantee you a stay in the local CSU.

You're not going to sustain brain damage; you're either going to die or sleep it off. The only possible exception would be if EMS resuscitated you after you had already sustained an anoxic brain injury, secondary to respiratory arrest. There's only about a 10 minute window for this opportunity after you have stopped breathing.

Of course you can refuse; hospital staff aren't the police. A simple "I'm not answering any questions about that" will suffice... frankly it's not their fucking business, and it has nothing to do with your medical management. For that matter, you're not obligated to answer questions from the police, either... in fact, I recommend that you don't, especially with police. In that case, your answer is, "I dont answer questions without a lawyer," and repeat as necessary.
