I read through Stans thread and i cant get any Metoclopramide in the u.k without prescription, i searched for buccastem which was what was suggested but it appears its no longer availble anywhere, i am also taking citalopram, ive checked and it doesnt appear that its something that will interact with these other drugs but im no expert so if anyone can confirm i'd be greatful, i assume from the various threads ive read that Meto isn't absolutely nesseary its just to make it easier and the fasting/antacid are the most important parts, i have diazapam to relax/put me to sleep, im just trying to put evereything else together

High recommend antiemetics. Heard plenty on how much SN sucks to drink and whilst SN has a high recovery rate for failure, is it something you wanna risk? Any issues with getting meto on a prescription? Commonly used for migraines.

There are a couple of anti-emetics that i think would be just as effective at Metoclopramide for nausea like dexamethasone or Haliperidol or even Odansetron and Levomapromazine. Each of these medications are used routinely in clinics where people are being treated for Cancer. My mum was effectively treated using one or sometimes two of these anti-emetics for her nausea post chemotherapy.

(1) General - What's important

the fasting/antacid are the most important parts
  1. Antacid is the least important part
  2. Fasting can be relaxed / flexible -- see here
  3. Diazapam is excellent to prevent anxiety , calm down , induce drowsiness , all and all make it peaceful (also decreases other symptoms)
  4. Regular painkillers are also helpful against headache

(2) Alternatives -- Discussion

would be just as effective at Metoclopramide for nausea like dexamethasone or Haliperidol or even Odansetron and Levomapromazine. Each of these medications are used routinely in clinics where people are being treated for Cancer.
CINV is not the same as toxic SN . Meto is really the gold standard and others may help nausea but are not systematic . This doesn't negate alternatives completely but it suggests those should be used only as last resort .

Other AEs?Don't use –
  • Ondansetron/Zofran
  • Diphenhydramine/Benadryl
  • Dimenhydrinate/Dramamine
Serotonin or Histamine only are not as effective.
Read guide ; List of AE by type – dopamine antagonists (Wikipedia)
Why not Ondansetron?"Metoclopramide is used by Dignitas and i think it's the recommened antiemetic in OD, the second one being domperidone, both dopamine antagonists. Ondansetron (Zofran) is a serotonin receptor antagonist and it's used for cancer patients undergoing chemotherapy because the irritation of the GI mucosa by the medication used in chemotherapy (which is cytotoxic and increase the levels of serotonin in the blood) are transmitted through the vagal nerve to the chemoreceptor trigger zone via activating serotonin receptors (5-HT3). It has no effect on dopamine receptors. "
In simple words?Need broad systematic AE targeting both CTZ (brain) and stomach plus prokinetic.
How vomiting worksComplex interactions:
  • Chemoreceptor Trigger Zone
  • Vomiting Centre
  • Nucleus Tractus Solitarius
  • GI tract chemoreceptors
See graphic schema .

(3) Solutions

@JamesF , as suggested by table , Domperidone is as effective as Metoclopramide (though the latter is preffered) and is OTC in Asia . As such it can be purchased via big auction sites . This is pretty much self explanatory and feasible . Metoclopramide may be purchased by some Asian-based online pharmacies etc , which may look dubious but actually deliver the goods (I won't discuss it further but some members know).
