As someone who struggled with many many doctors over the years who didn't care to do the correct tests on me for over a decade. The best advice I can probably offer you is to try to start seeking medical treatment Inside a major city preferably a teaching hospital. I say this because average doctors really have very little incentive to properly diagnose. Hospitals that are actively involved in research however are very motivated to treat and solve conditions.
I got lucky that I live about an hour away from Chicago and I found a doctor actively contributing to the medical research in literature concerning small fiber neuropathy. He was able to start helping me treat my condition. Unfortunately he moved to another state recently and things that become much harder. Someone gave me advice that I should go into the city before I ever got diagnosed and it was the best advice they ever gave me.
