Does anyone personally know someone who attempted with a firearm and failed? If so what was the aftermath? What did they do wrong?

Personally, no. I did see a fair bit of medical exams of failed firearm attempts and the common denominator has always been that the bullet's force mostly missed any vital areas. Usually people who don't hit the brain stem or temple. They end up with blown off faces, and they don't bleed out before they're found and "rescued".

Not really. But could have failed.
I had a friend that killed themself via shotgun.
However by account of their sibling who was in another part of the house. A first shot was heard, by time they ran and reached the room a second shot occured. She realized what happened and did not enter and called emergency services.
What was left of the head suggested the first shot was a flinch that blew off the side of his face. He then finished it with a second shot. Seems like he had the barrel in his mouth, but the flinch shifted his aim to the side. Second shot was between the eyes. So not much head left.
There was maybe 10 seconds between the two shots. 10 seconds of agony I would presume. Had he not been able to pull off the second shot, he probably would have survived minus a chunk of his face.
Firearms are a high success rate, but its still possible to screw up.
It would still be my choice if I could get one..

that's a lot of willpower to survive the 1st shot and then fire again. They really wanted to die

There was this guy i saw who had a profile on tiktok years ago who attempted w a shotgun and somehow survived, his face was permanently disfigured but he did overcome his depression eventually. I recall he actual had a decent quality of life post recovery, although his face being completely fucked.
It's possible but very very rare
There was a man in my hometown who shot himself in the lower jaw with a rifle. The lower jaw was almost completely destroyed, but he survived. It was a frightening sight as a child.

I have no idea how he pulled it off. I'm not sure if he would have survived after the first shot. Paramedics would have arrived within 15 mins, so they might have saved him.

Yes, they aim too high and hit towards the front of the roof, and just end up blasting the front of their face off. It's still extremely rare to survive even that because the blood loss is substantial, and there's extreme brain damage either way. I should note that "survive" here only means the body still has vital signs. There might not be anybody home at that point.
