Jon-Erik Hexum, Norwegian/Icelandic actor, played on set with a gun loaded with fake bullets in 1984 and pointed it to his head as a joke. He decided to shoot and the shot was fatal. 6 days of agony with surgery and then death. A gun doesn't always need real bullets to kill.
