Physical - Having eye floaters, Itchy legs and cavity sore teeth.

Mental - Back in Oct/Nov 2014 when I felt like suicide impulsively then attacked someone

Physical: I guess it must have been painful when i was around 2 years old. I fell to the ground on my head from a tall chair. I don't remember the episode.

Mental: A lot derived from my social anxiety disorder. Tachycardia, sweating, mental overwhelming. Remember one time in university, a moment before i had to do an oral presentation, i was so overwhelmed with negative ideas i thought my heart was going to explode.
