Not getting reliable notifications on updates?

Posts I am watching/commenting on don't seem to be appearing in notifications reliably.

I feel like this has been happening to me for a while. I usually check my Watched Threads to see if I'm missing anything.
I don't know if that will help anyone but maybe try that.
